I will bill your insurance directly.  Coverage of services will depend on your individual policy.  You are responsible for any copays, co-insurance, high deductibles, or other expenses not covered by your insurance plan at the time of service.  You are strongly encouraged to contact your insurance company prior to your appointment to gather information about your coverage; it is the responsibility of the patient to understand their coverage.  Procedure codes most often billed to your insurance company for evaluations include 90791 or 96116, 96132/96133 or 96130/96131, and 96136/96137.  For therapy services, most common procedure codes are 90791 and 90837.  It can be helpful to contact your insurance with these specific procedure codes when asking about coverage.

Some patients may choose to not involve their insurance company in order to maintain their privacy.  I accept all major credit cards, HSA, cash, and check.

My rates are as follows:

• Initial 20-minute consultation: Free

• Initial intake session: $225

• Subsequent therapy sessions (50-55 minutes): $185
